People who have lost a travel one often try to make some meaning of their loss. The authors explore the ways people try to make meaning of loss, the factors that predict difficulty in making meaning, and the emotional outcomes of finding meaning. They also contrast the process of finding meaning with finding some benefit in the loss, even if meaning cannot be found. Our discussion centers on a study of 205 bereaved people who were interviewed before their loss and 1, 6, 13, and 18 months after their loss. The authors draw conclusions from this work not only for bereavement theories but also for general theories of adjustment in social and personality psychology.

The field of grief therapy is currently in a state of conceptual revolution, opening the prospect of reconfiguring our understanding of the human experience of loss along constructivist lines. In this article I outline some of the tenets of such an approach, proposing that the reconstruction of a world of meaning is the central process in grieving. I then present several narrative strategies for assisting bereaved people in making meaning of loss, and discuss indications, illustrations, variations, and precautions pertinent to each.

Grounded theory analysis was used to generate an explanation of the phenomenon of meaning reconstruction in the experience of 10 bereaved mothers. The theory that emerged included three phases in the process of meaning reconstruction: discontinuity, disorientation, and adjustment. The participants reinterpreted the meaning structures they had held prior to their child's death in order to give meaning to the death. The nature of the prior meaning structure merged as the core variable. The ability to restore meaning after the death of a child was clearly linked to the prior existence of a meaning structure that could account for and "place" the child's death. The findings suggest that the process of meaning reconstruction is a unique aspect of the grieving experience that cannot be explained by existing theories of grief. Recognition of the complexity of the process has implications for community members and professionals who interact with the bereaved parent.

A comprehensive quantitative review of published randomized controlled outcome studies of grief counseling and therapy suggests that such interventions are typically ineffective, and perhaps even deleterious, at least for persons experiences a normal bereavement. On the other hand, there is some evidence that grief therapy is more beneficial and safer for those who have been traumatically bereaved. Beginning with this sobering appraisal, this article considers the findings of C. G. Davis, C. B. Wortman, D. R. Lehman, and R. C. Silver (this issue) and their implications for a meaning reconstruction approach to grief therapy, arguing that an expanded conception of meaning is necessary to provide a stronger basis for clinical intervention.

Three assumptions guiding research and clinical intervention strategies for people coping with sudden, traumatic loss are that (a) people confronting such losses inevitably search for meaning, (b) over time most are able to find meaning and put the issues aside, and (c) finding meaning is critical for adjustment or healing. We review existing empirical research that addresses these assumptions and present evidence from a study of 124 parents coping with the death of their infant and a study of 93 adults coping with the loss of their spouse or child to a motor vehicle accident. Results of these studies indicate that (a) a significant subset of individuals do not search for meaning and yet appear relatively well-adjusted to their loss; (b) less than half of the respondents in each of these samples report finding any meaning in their loss, even more than a year after the event; and (c) those who find meaning, although better adjusted than those who search but are unable to find meaning, do not put the issue of meaning aside and move on. Rather, they continue to pursue the issue of meaning as fervently as those who search but do not find meaning. Implications for both research and clinical intervention are discussed.

This paper, utilising a narrative approach, aims to describe the experiences of men whose partner had experienced pregnancy loss, based on data from Northern Ireland. The methodology was based upon observation within pregnancy loss self-help groups and in-depth interviews with 14 men who attended the groups. The study also included interviews with 32 midwives and nurses, with the intention of examining attitudes within the medical context towards bereaved fathers. The impact of pregnancy loss on male partners has been largely overlooked in academic research. When a baby dies before birth the loss can be devastating for fathers yet, very often, the world that surrounds them tends to discount their loss, and emotional support and cultural rituals that are normally available to other bereaved individuals are often absent for this group of men. Previous research has shown that men are expected to be emotionally strong in order to support their partner. The present study will show that the perception that men have only a supportive role in pregnancy loss is unjustified, as it ignores the actual life-world experiences of the men, and the meanings they attach to their loss, in what may be a very personal emotional tragedy for them where they have limited support available. The study uncovered several recurring themes including self-blame; loss of identity; and the need to appear strong and hide feelings of grief and anger. There is consideration of the need for hospital staff and the wider community to acknowledge the male partner's grief as being a valid response to the bereavement suffered.

This article explores the concepts of narrative as story, of storytelling, and of the narrative approach to qualitative research. Within this, I will also examine the social nature of narrative and the implications of this for research. I will look at the process of conducting a narrative study and the implications for participation in such a project and for the researcher investigating a phenomenon through this frame. In particular, the need to create stories to make order of disorder and find meaning in the meaningless is particularly relevant to the study of grief. In looking at the process of conducting a narrative study, questions about what can be analyzed and how it might be presented, some strengths and limitations of the narrative approach, and ethical questions also are considered.

BACKGROUND: Hope is a multidimensional concept that is important for all parents of children with cancer. However, most work has focused on advanced cancer and poor prognoses. We examined hopes of all parents of children with cancer longitudinally during the first year of treatment. PROCEDURE: Prospective, longitudinal, questionnaire-based cohort study of parents and physicians of children with cancer at two academic pediatric hospitals. Parents reported on general sense of hopefulness and specific hopes at time of diagnosis (N = 374); a subset of parents (N = 164) were followed longitudinally at 4 and 12 months. RESULTS: Fifty-five percent of parents (N = 206/374) reported being extremely hopeful in general at baseline. Hopefulness did not significantly change over time, and most parents (51-58%) reported being extremely hopeful regardless of prognosis (P = 0.66). Most parents (N = 327/356) considered hope for cure to be an extremely important source of hope; most also reported hope that the child would feel loved (N = 328/356), that the child would have the best possible quality of life (N = 316/356), and that they would always do all they could for the child (N = 300/356). Hope for cure was slightly lower among parents of children with less than a 50% chance of cure at baseline (N = 53/63) when compared to those with better prognoses (moderately likely cure, N = 76/78; very likely cure, N = 198/215) (P = 0.02). CONCLUSIONS: Many hopes contribute to parental hopefulness, not just hope for cure. This hopefulness persists over time, even when the prognosis is poor. Clinicians should focus on supporting the myriad hopes that contribute to overall hopefulness.

The aim of this study was to focus on the relationship of forgiveness toward others, self-forgiveness, and anger toward God after the loss of a child. A sample of 84 grieving parents completed a battery of questionnaires. Self-forgiveness was found to be the strongest predictor of avoiding negative psychological adjustment after the loss (defined by the variables of normative and complicated grief, depression, anxiety, and anger) and the strongest predictor of positive psychological adjustment after the loss (defined by the variables of life meaningfulness, sense-making, benefit finding, and meaning reconstruction).
