A 3 1/2-year-old boy presented at three months of age with an acute episode of lethargy, somnolence, hypoglycemia, hepatomegaly, and cardiomegaly, which responded poorly to restoration of the blood sugar level to normal. The absence of ketonuria during subsequent episodes of severe hypoglycemia prompted a search for a defect in fatty acid oxidation. Plasma carnitine (2.0 to 5.0 mumol per liter), muscle carnitine (0.01 to 0.02 mumol per gram, wet weight) and liver carnitine (0.021 to 0.065 mumol per gram, wet weight) were all less than 5 per cent of the normal mean. During a 36-hour fast, ketones were barely detectable. Prolonged treatment with oral carnitine over a six-month period resulted in increased muscle strength, a dramatic reduction in cardiac size, relief of cardiomyopathy, partial repletion of carnitine levels in plasma and muscle, and complete repletion in the liver. Systemic carnitine deficiency is an easily treatable cause of recurrent Reye's-like syndrome. Its diagnosis requires measurement of carnitine levels.

In rats that were fasted for 2 to 3 days there was a decline in hypothalamic, but not pituitary, beta-endorphin. There was no change in pituitary or hypothalamic adrenocorticotropin content as a result of fasting. Endogenous opiates may be involved in physiological adaptation to fasting.

In a series of patients with the Lesch-Nyhan syndrome a pharmacologic attempt was made to alter the balance of biogenic amines in the central nervous system by increasing the level of serotin. Patients were treated with 5-hydroxytryptophan in combination with a peripheral decarboxylase inhibitor carbidopa and with imipramine. Most patients had a striking alteration in self-mutilative behavior. However, within 1–3 months each became tolerant and this pharmacologic effect could not be produced again even a year later. These observations suggest that the characteristic behavior of the patient with the Lesch-Nyhan syndrome is related to neurotransmitter balance, and that there may be ways to modify it.

The Grief Experience Inventory (GEI) and the MMPI were used to assess bereavement reactions in 102 newly bereaved individuals; 107 controls were also assessed. Intensities of bereavement reactions were compared across three types of deaths experiences, i.e., spouse, child, and parent. Significantly higher intensities of grief were noted in parents surviving their child's death. A distinct number of physiological symptoms were noted in the bereaved group as compared to controls. Frequent church attenders were more likely to respond with higher optimism and social desirability but more repression of bereavement responses than were less frequent church attenders. Income did not appear to contribute negatively to bereavement itself but rather to the constellation of debilitating variables which surrounds those with low income. There were no differences in bereavement intensities between those who survived a chronic-illness death as compared with sudden death situations.

The body composition of 23 patients with cachexia resulting from malignant or benign inflammatory disease was studied. Skinfold thickness and radioisotope tracer measurements enabled us to estimate total body fat, lean body mass, the ratio of fat loss to lean body loss, erythrocyte and plasma volumes, total body water, extra-, and intracellular water volumes, total body potassium, and intracellular potassium concentration. The total body fat and lean body mass information suggests that the body composition in cancerous cachexia differs from that in cachexia caused by benign inflammatory disease, in that lean tissue appears to be better conserved in the former. The radioactive tracer studies showed that the cachectic patients were anemic, potassium-deficient, and overhydrated; however, fluid partitioning into extra- and intracellular compartments was normal.
