When a child dies, the survivors-both parents and the remaining children—must adapt to a new reality. The immediate effect of sibling death is to precipitate grief and to increase the psychologic vulnerability of the remaining children. Communication patterns within a family in part determine the ease or difficulty with which members are able to live with the death of a child. This article discusses a family-centered approach toward support of siblings in bereavement. Strategies that assist surviving family members to mobilize their own strengths are presented. A review of resources pertaining to sibling loss is included.

This study focuses on descriptive accounts of one Macmillan nurse's work, as provided by key individuals coming into contact with this specialist professional service. Twenty respondents (eight patients, five carers, five district nurses and two general practitioners) were interviewed using a variation of the critical incident technique. Data were analysed in terms of meaningful observed events (critical happenings) that were perceived as effective or ineffective with respect to the delivery of high-quality nursing care. Some variations were found between groups of respondents in their perception of the nurse's role. However, there was a general emphasis on the possession of specialist knowledge of terminal cancer care and the positive impact of interventions to both the patient and to lay and professional carers. The critical incident technique was found to be a valuable method for eliciting detailed accounts of the work of the nurse in this specialized field of practice.

The purpose of this study was to assess the receptiveness of parents to information given about their child's life threatening illness. Three months after the child's diagnosis, an independent interviewer, using a structured questionnaire, interviewed the parents about what and how they had been told, and assessed the stage of parental coping reached at that time. All patients were treated at a regional paediatric oncology centre (RHSCE) and the interviews were conducted preferably in the family home (61%), or in a 'quiet' hospital room (39%). Twenty-five, unselected, consecutive patients with cancer and leukaemia diagnosed and treated at the RHSCE and their families were recruited into the study (1988-1989). One child died during the first three months and one single parent family refused co-operation. Eighteen of the 23 interviews conducted were with both parents, and five were with single parents (all maternal). All 23 sets of parents admitted deep shock and devastation on hearing the initial diagnosis, with 12 sets feeling that they had taken in little or none of the information given. A long interview was conducted a few days after the initial talk with the consultant. The parents of four children (17%) denied that the long interview had occurred whilst 10/19 who remembered it expressed specific lack of understanding of some or all details. All families remembered a large number (10-20) of subsequent talks with a wide range of staff, but 14/23 felt that some of the information was still confusing or conflicting, and 9/14 did not want to ask for further clarification, principally because they did not want to hear more bad news. The majority felt the child understood little of what was going on. At the study interview, most parents were assessed as still being very anxious about their child's illness, whilst two couples, one single mother and one father, were content with what and how they had been told about their child's illness and were in a state of emotional homeostasis. It is concluded that communication of bad news is a two-way process requiring skilled medical staff, but also a receptive audience. The emotional state of the parent determines his or her ability to hear and comprehend the information given. The results imply that repetition and clarification at consultation interviews is required until parents are emotionally able to hear, accept and comprehend complex news. Written material, taped interviews and simple videos can assist in this process.

A randomized, double-blind, prospective study was performed to determine the effects of perioperative administration of morphine or methadone on postoperative analgesic requirements and pain scores in 35 children aged 3 to 7 years undergoing major surgery. After a standardized induction of anesthesia, methadone or morphine, 0.2 mg/kg, was blindly administered, and supplemental doses were titrated to achieve comfort in the recovery room. Pain was assessed during the next 36 hours with a combination of validated behavioral and self-report measures. Patients in the methadone group required fewer supplemental opioid analgesic drugs during the next 36 hours, and reported lower pain scores. No patient had prolonged emergence from anesthesia, and no patient required naloxone or postoperative ventilatory assistance. No major adverse events occurred. We conclude that perioperative intravenous administration of methadone is an effective, inexpensive, and technologically simple means for providing prolonged analgesia for children after surgery.

OBJECTIVE--To develop and pilot a method for conducting an audit of deaths in general practice by the critical incident technique. DESIGN--Prospective use of the technique within a primary health care team, with the aid of a facilitator, to analyse the events surrounding patients' deaths. SETTING--One inner city academic general practice. PARTICIPANTS--Practice team, comprising general practitioners, trainee, practice manager, practice nurse, and attached health visitor and district nurses. MAIN MEASURES--Identification and classification of critical incidents associated with the case studies of eight recently decreased patients in the practice and subsequent impact on the practice. RESULTS--Among the eight case studies, 57 critical incidents were identified (mean 7.1 per case, range 2 to 15). A failure of communication was the most common factor identified in incidents giving rise to concern, but positive factors in patient care were also identified. Changes in practice included developing protocols for follow up of bereaved relatives and carers and a checklist to ensure completion of administrative follow up tasks resulting from the patient's death; cases of recent deaths and terminally ill patients were reviewed monthly. The practice team found the method acceptable and felt that the discussions had provided useful opportunities for reflecting on their role in patient care. CONCLUSIONS--The critical incident technique fulfils the needs of an audit of deaths in general practice; however, further evaluation based on more cases from different practices is now required.

Objective: To examine the effects of advance directives on medical treatments and on patient satisfaction and well-being and to determine whether the enhancement of patient autonomy through advance directives provides a more ethically feasible approach to cost control than does the imposition of limits through rationing.; Design: Randomized, controlled trial.; Setting: University and Veterans Affairs medical center.; Patients: Two hundred and four patients with life-threatening illnesses, 100 of whom died after enrollment in the study.; Intervention: Patients randomly assigned to the experimental group were offered the California Durable Power of Attorney (a typical proxy-instruction directive), and patients assigned to the control group were not offered the advance directive. Hospital admissions were monitored to assure that a summary of the document was present in the active medical record at each hospitalization.; Measurements: Cognitive function, patient satisfaction, psychological well-being, health locus of control, sense of coherence, health-related quality of life, receipt of medical treatments, and medical treatment charges.; Results: No significant differences were found between advance-directive and control groups regarding psychosocial variables, health outcome variables, and medical treatments or charges. Patients offered an advance directive had an average hospital stay of 40.8 days (95% CI, 32.2 to 49.4 days), compared with an average of 33.1 days (95% CI, 26.0 to 40.2 days) for controls. Patients offered an advance directive were charged an average of $19,502 (95% CI, $13,030 to $25,974) for medical treatments in the last month of life compared with $19,700 (95% CI, $13,704 to $25,696) for controls.; Conclusions: Despite claims that public demand for longer life accounts for rising medical costs, most surveys suggest that patients are calling for less, not more, of the expensive, high-technology treatment often used in terminal phases of illness. Executing the California Durable Power of Attorney for Health Care and having a summary copy placed in the patient's medical record had no significant positive or negative effect on a patient's well-being, health status, medical treatments, or medical treatment charges.;

We used data from the National Longitudinal Survey of Youth (NLSY; 1987) to examine the relationship between having a chronic physical health condition during youth (ages 14 to 21), and the subsequent transition to adulthood among a nationally representative sample (10,485), followed from 1979 to 1986. We controlled for theoretically important baseline characteristics of the youths and their families in multivariate regression models examining educational attainment, marriage, income, unemployment, and self-esteem. Analyses suggest that adolescents and young adults with chronic physical health conditions are at a slightly higher risk of problems in early adult life, but these effects are overshadowed by more potent social, economic, and demographic characteristics of both the youths and their families of origin. These results were unexpected; we had hypothesized a wide variety of mechanisms whereby individuals with chronic conditions would not perform as well as their healthy peers in

A taxonomy of the adolescent sibling bereavement process was developed from the responses of 157 bereaved adolescents to the question "If you could ask or tell your dead sibling something, what would it be?" The taxonomy had six mutually exclusive categories: regretting, endeavoring to understand, catching up, reaffirming, influencing, and reuniting. Pervasive throughout the taxonomy was the theme of "ongoing attachment"; that is, the emotional and social bond with the deceased sibling remains continuous throughout the bereavement process. The notion of ongoing presence illustrates the timelessness of adolescent bereavement and the infiniteness of the sibling bond. The phenomena of "ongoing presence" and timelessness challenge current medical science conceptualizations of bereavement that postulate that grief is time bound and that healthy recovery depends on severance of emotional bonds with the deceased. Qualitative and quantitative studies are needed to develop a substantive theory of adolescent sibling bereavement that is developmentally appropriate, accounts for variability by cause of and circumstances surrounding the death, and is culturally relevant.

A child's death can have a profound and lasting effect on surviving siblings. Early researchers and clinicians have suggested that siblings were at risk for serious psychopathology. However, in later studies, researchers found that although the grief process was emotionally difficult, siblings may have grown and matured as a result of living through the loss. The experience of sibling bereavement is relatively unexamined in the literature; the long-term effects of such an experience have received even less attention. In this study, grounded theory techniques for the analysis of qualitative data were used to analyze a series of intensive, semistructured interviews with 12 adults who, in their early adolescence, lost a sibling. Long-term outcomes included psychological growth, a sense of feeling different, and withdrawal from peers. The study presents a theoretic scheme relating these outcomes. The sense of personal growth and maturity arouses feelings of being different from peers, and may result in an intolerance of the developmentally appropriate behaviors demonstrated by peers. Some siblings respond to these feelings by withdrawing from their peers at a time when peer relationships are critical to completing developmental tasks. For such siblings, feelings of sadness and loneliness become long-term.

The authors explored the long-term consequences for adolescents of surviving a sibling afflicted with cystic fibrosis. Seventy-five adults were interviewed whose siblings had died from cystic fibrosis in the 1960s and 1970s. Subjects were drawn from case records at Children's Hospital, Boston; Children's Hospital, Oakland; and the University of California, San Francisco. For quantitative analysis, a sample wasdrawn of25 survivors who were under the age of 19 years at the time of the death. Anxiety and depression scales derived from the Hopkins checklist were used, and a 3-point scale developed for guilt. There were no significant relationships between sibling adaptation and number of siblings lost, birth order and gender, family size, or time since death of sibling. The age of the surviving sibling at the time of the death was statistically significant. Those who had been between 13 and 17 years of age at the time of the loss expressed the most symptoms. They were troubled by a global sense of guilt, guilt over their handling of the sibling's illness and death, and survival guilt. They also manifested global anxiety, bodily concerns and feelings of vulnerability, fear of intimacy, excessive concern for others, somatic complaints, and sleeping difficulties.

The long-term effects of bereavement were assessed 7 to 9 years after death by interviews with 31 adolescent siblings from 21 families who participated in a longitudinal study of home carefor children with cancer. Categorical analysis of the semistructured interviews revealed that the majority of the adolescents viewed the experience as having fostered their personal or family growth. However, about I in 6 regarded the effects of a sibling death as continuing to have a negative impact on their lives. Factors associated with a more positive outlook were good communication in the family, ability to share the death experience with others, expression of pleasure in sibling's company, and reliance on the family for emotional support. Factors associated with a more negative outlook were withdrawal from family interaction, inability to use the family as a source of support, and difficulty in discussing one's experience with death.

Qualitative researchers rely — implicitly or explicitly — on a variety of understandings and corresponding types of validity in the process of describing, interpreting, and explaining phenomena of interest. In this article, Joseph Maxwell makes explicit this process by defining five types of understanding and validity commonly used in qualitative research. After discussing the nature of validity in qualitative research, the author details the philosophical and practical dimensions of: descriptive validity, interpretive validity, theoretical validity, generalizability, and evaluative validity. In each case, he addresses corresponding issues of understanding. In conclusion, Maxwell discusses the implications of the proposed typology as a useful checklist of the kinds of threats to validity that one needs to consider and as a framework for thinking about the nature of these threats and the possible ways that specific threats might be addressed.

OBJECTIVE: To compare clinicians' ratings of therapeutic effectiveness when different trial end points were presented as percent reductions in relative compared with absolute risk and as numbers of patients treated to avoid one adverse outcome. DESIGN: Survey, with random allocation of two questionnaires. SETTING: Toronto teaching hospitals. RESPONDENTS: Convenience sample of 100 faculty and housestaff in internal medicine and family medicine. INTERVENTION: One questionnaire presented results for three end points of the Helsinki Heart Study as separate drug trials using only absolute differences in events; the other showed the same end points as relative differences. Both questionnaires included a fourth "trial," showing person-years of treatment needed to prevent one myocardial infarction. MAIN OUTCOME MEASURE: The "trials" were each rated on an 11-point scale, from treatment "harmful" to "very effective." RESULTS: Respondents' ratings of effectiveness varied with the end point. Controlling for end point, ratings of effectiveness by the 50 participants receiving absolute event data were lower than those by 50 participants responding to relative risk reductions (P < 0.001); however, no end-point difference was more than 0.6 scale points. For a "trial" reporting that 77 persons were treated for 5 years to prevent one myocardial infarction, mean ratings were 2.3 or 1.8 scale points lower, respectively (both P < 0.001), than when the same data were shown as relative or absolute risk reductions. CONCLUSIONS: Clinicians' views of drug therapies are affected by the common use of relative risk reductions in both trial reports and advertisements, by end-point emphasis, and, above all, by underuse of summary measures that relate treatment burden to therapeutic yields in a clinically relevant manner.

Opioid peptides appear to be dynamic signaling molecules that are produced within the immune system and are active regulators of an immune response. Furthermore, the receptors for these peptides occurring on immunocyte membranes share characteristics with neuronal opioid receptors, including molecular size, immunogenicity, and the use of specific intracellular signaling pathways. Recent studies of the interaction of opioids with cytokines have indicated that opioid peptides are intimately involved within the immune system. Specifically, opioids, including 2-n-pentyloxy-2-phenyl-4-methyl-morpholine, naloxone, and beta-endorphin, have been shown to interact with IL-2 receptors (134) and regulate production of IL-1 and IL-2 (48-50, 135). Conversely, IL-1 has been shown to up-regulate opioid peptide binding in brain tissue (136). Furthermore, the induction of IL-1 by opioids has also been identified in the invertebrate Mytilus, indicating the evolutionary conservation of this relationship (137). These results seem to typify the intricate association between the immune and neuroendocrine systems through opioid pathways. It is predicted that future endeavors will use this relationship to diagnose and treat specific diseases that have at their basis neuroendocrine and immunologic imbalances.
